当代社会,企业组织在发展中普遍面临社会责任与经济效益“博弈”的问题, 经济学者们致力于探求企业组织社会责任与经济利益的平衡机制,以期实现二者 良性互动。公立医院作为公益事业单位,自成立以来既具有提供公共卫生服务, 保障居民卫生健康的公益性属性,也存在通过药品加成收入、服务收费途径追求 经济效益的积极性属性。在我国,公立医院在实际发展中需要兼顾公益性与积极 性。 2009年中共中央、国务院颁布了《关于深化医药卫生体制改革的意见》, 新一轮的医改正式启动,新医改要求公立医院改革要积极探索政事分开、管办分 开的有效形式,建立科学合理的管理制度及经营方式,提高医院的服务效率,降 低医疗卫生费用。在明确医院的公益性前提不动摇,转变公立医院收入结构,由 药品加成收入、服务收费、财政补贴三方面转变成服务收费、财政补贴两方面, 给患者减轻了一部分负担。 2014年S省正式启动了医疗卫生改革,本文以新医改背景下Z区人民医院 为例,分析研究2014至2018年Z区人民医院公益性与积极性均衡发展战略,根 据其近五年来的人事、财务数据和日常医院制定的管理政策规定,解析改革中Z 区医院如何实施公益导向下经营战略,为医院下一步发展提出有利建议。 本文在研究的过程中,主要涉及到以下三个方面的内容。第一,通过探究国 内外学者关于公立医院改革的研究文献综述,对公立医院改革的积极性与公益性 均衡策略进行阐述分析。第二,选取东部沿海省会城市中的Z区医院作为研究案 例,介绍Z医院基本情况以及在新医改中面临的问题与发展困境。第三,分析医 院的运行发展状况以及医院日常政策规定,探究讨论自2014年新医改政策实施 之后,医院发展受到的影响以及医院为进一步发展采取的有效措施方略。该研究 通过对Z医院进行分析研究,探讨了2014年本省新医改政策实施以来对医院的 直接影响,通过数据分析医院在应对改革中如何实施公益导向下经营战略,研究 成果有助于在后期单位日常发展建设中,为医院发展提出有益的新的借鉴。 关键词:公立医院;公益性;积极性;经营战略 1 山东建筑大学论文 Researchonthemanagementstrategyofpublichospitalsunderthe guidanceofpublicwelfareobjectivesunderthebackgroundofmedical reform--TakingZDistrictPeople'sHospitalasanexample Abstract Incontemporarysociety,corporateorganizationsgenerallyfacetheproblemof"game" betweensocialresponsibilityandeconomicbenefitsintheirdevelopment.Economistsare committedtoexploringthebalancemechanismbetweencorporatesocialresponsibilityand economicinterests,inordertoachieveabenigninteractionbetweenthetwo.Aspublicwelfare institutions,publichospitalshavenotonlyprovidedpublichealthservicestoprotectthepublic healthofresidents,butalsohaveprofitableattributesthatpursueeconomicbenefitsthroughdrug add-onincomeandservicecharging.InChina,publichospitalsneedtotakeintoaccountboth publicwelfareandprofitabilityinactualdevelopment. In2009,theCentralCommitteeoftheCommunistPartyofChinaandtheStateCouncil promulgatedthe"OpinionsonDeepeningtheReformoftheMedicalandHealthSystem".Anew roundofmedicalreformwasofficiallylaunched.Thenewmedicalreformrequiresthatpublic hospitalreformsshouldactivelyexploreeffectiveformsofseparationofgovernmentaffairsand management,andestablishscientificandrationalmanagementsystemandmethodstoimprove hospitalserviceefficiencyandreducemedicalandhealthexpenses.Intheclearpublichospital's publicwelfarepremise,theincomestructureofpublichospitalswillbetransformed.Thethree aspectsofdrugadd-onincome,servicechargesandfinancialsubsidieswillbetransformedinto servicechargesandfinancialsubsidies,whichwillalleviatesomeoftheburdenonpatients. In2014,SProvinceofficiallylaunchedthemedicalandhealthreform.Thisarticletakesthe ZDistrictPeople'sHospitalunderthebackgroundofthenewmedicalreformasanexampleto analyzethebalanceddevelopmentstrategyofpublicwelfareandprofitabilityoftheZDistrict People'sHospitalfrom2014to2018.Accordingtoitspersonnel,thefinancialdataandthe managementpoliciesformulatedbythedailyhospitalsinthepastfiveyears,thisarticleclarifies howtheZhospitaltakesintoaccountthepublicwelfareandprofitabilityinthereform,and providesfavorablesuggestionsforthenextdevelopmentofthehospital. 2 山东建筑大学论文 Intheprocessofresearch,thispapermainlyinvolvesthefollowingthreeaspects.Firstly,by exploringtheliteraturereviewofscholars'reformsonpublichospitalsathomeandabroad,this paperexpoundsandanalyzestheenthusiasmofpublichospitalreformandthepublicwelfare equilibriumstrategy.Secondly,theZDistrictPeople'sHospitalinthecapitalcitiesoftheeastern coastalprovinceswasselectedasaresearchcasetointroducethebasicsituationofitandthe problemsanddevelopmentdilemmasfacedinthenewmedicalreform.Thirdly,thepaper analyzestheZDistrictPeople'sHospital’soperations,thedevelopmentandthedailypolicy provisions,andexploretheimpactsofitsdevelopmentsincetheimplementationofthenew medicalreformpolicyin2014andtheeffectivemeasuresadoptedbythehospitalforfurther development.ThroughtheanalysisandresearchofZhospital,thisstudyexploresthedirect impactsonthehospitalsincetheimplementationofthenewmedicalreformpolicyin2014. Throughdataanalysis,thisstudyexploreshowthehospitalbalancesthepublicwelfareand economicbenefitsinthereform.Theresearchresultswillhelptoprovideusefulnewreferences forthehospital’sdevelopmentinthelaterstage.
