摘要
随着我国经济的快速发展与近两年新冠疫情“黑天鹅”事件的出现,普通民众对
身体健康的关注与医疗服务的需求都与日俱增。医院,尤其是公立医院承担着绝大多
数的医疗服务任务,但是其自身的发展速度难以满足国家和民众的需要。在此背景下,
PPP 模式得到了大力的推广,PPP模式也开始运用到医疗行业的发展中去。PPP 模式
作为串联社会资本和政府部门的纽带,在推动行业发展,缓解地方债务问题的同时也
存在各种各样的风险,财务风险就是众多风险因素之一,也是较为重要的风险因素。
财务风险的爆发会对 PPP 项目的实施产生影响,严重情况下则会直接导致项目的失
败,给政府部门和社会资本方利益造成损失。所以在医疗卫生行业 PPP 项目的开展过
程中,如何对项目的财务风险进行有效控制,是决定项目能否顺利实施的关键。
基于上述问题,本文主要运用案例分析法、问卷调查法以及专家访谈法,以东部
某省一家市级公立医院——N市中医院的PPP 项目为实例进行研究。首先,本文梳理
总结了国内外PPP 模式与 PPP 项目风险管理的研究现状,以公共管理理论、委托代理
理论、风险管理理论、财务风险管理理论为理论基础,再结合我国 PPP 模式的相关政
策,为后文的研究打下基础。其次,本文通过对医疗类PPP 项目的行业现状、项目概
况以及财务风险成因的研究,从风险识别、风险评价、风险控制三个维度,构建出N
市中医院迁扩建 PPP 项目财务风险管理体系,从筹资风险、投资风险、成本超支风险、
收益分配风险四个方面对该类项目的财务风险进行了详细分析与评价。最后,针对上
述四个方面的财务风险提出相应的风险控制对策及优化建议。
通过上述研究,本文得出主要结论:(1)该项目的财务风险成因主要包含内外
部两个方面。其中内部原因是引发财务风险的主要因素。(2)该项目的财务风险主
要体现在筹资、投资、成本超支和收益分配四个方面,具体包含 12 项具体的财务风
险因素。(3)在各类财务风险中,筹资风险为项目的主要财务风险,主要体现在筹
资结构不合理、融资手段单一等方面,需要引起各参与方的高度重视。(4)根据对
案例的具体分析,结合实际情况从四个方面提出相应的财务风险控制对策,并针对医
疗行业 PPP 项目财务风险控制提供参考性建议。

ABSTRACT
With the rapid development of Chinese economy and the emergence of the black
swan event of coronavirus pandemic in the past two years, the concern of ordinary
people for physical health and demand for medical services are increasing day by day.
Hospitals, especially public hospitals, undertake the vast majority of medical services,
but their own development speed is difficult to meet the needs of the country and the
public. In this context, the PPP model has been vigorously promoted, and it has also
begun to be applied to the development of the medical industry. PPP mode, as a link
connecting social capital and government departments, promotes the development of
industries and alleviates local debt problems, but also has a variety of risks. Financial
risk is one of the many risk factors, which is also a relatively important risk factor.
The outbreak of financial risk will affect the implementation of PPP projects, and in
serious cases, it will directly lead to the failure of the project, causing losses to the
interests of government departments and social capital parties. Therefore, in the
process of carrying out PPP projects in the medical and health industry, how to
effectively control the financial risks of the projects is the key to the smooth
implementation of the projects.
Based on the above problems, this paper mainly uses case analysis method,
questionnaire survey method and expert interview method to study the PPP project of
N City Traditional Chinese Medicine Hospital, a municipal public hospital in an
eastern province. First of all, this paper summarizes the research status of PPP model
and risk management at home and abroad, based on the theory of public management,
principal-agent theory, risk management theory and financial risk management theory
as the theoretical basis, and then combines the related policies of PPP model in our
country to lay the foundation for the research in the future. Secondly, through the
study of the industry status, project overview and financial risk causes of medical PPP
projects, this paper constructs a risk management system for PPP projects of
traditional Chinese medicine hospital relocation and expansion in N city from three
dimensions of risk identification, risk evaluation and risk control. The financial risk of
this kind of project is analyzed and evaluated in detail from four aspects: financing
risk, investment risk, cost overrun risk and income distribution risk. Finally, according
to the above four aspects of the financial risk put forward the corresponding risk
control countermeasures and optimization suggestions.
